Go to “Encounter Information” and “Visit Info”

Verify the information in the following fields:

  • Chief Complaint: enter “Health Screening”
  • Confirm Payment type is Other Coverage and Voucher
  • Special Offer: “Amgen Cholesterol Screening Voucher”
  • Obtain history, VS (weight, height, BMI, WC, BP) and CVD risk assessment, as indicated
    • Copy Documentation from Risk Assessment into Progress notes
  • Complete Lipid POCT ONLY
  • Use SmartSet: “Health screening”
  • Scan signed voucher into Scans and Consents as type: Special Offer
  • Complete visit/document in EPIC
